I love this app, I had bought tvGuide thinking I would rather view what's on using their method, but I came crawling back to Ice pretty quickly.
If it wasn't too sensitive, I would like to change the day by swiping my finger left or right across the screen. That would also require 24 hours to be shown instead of the 8 or so shown currently.
If there are problems with displaying more than the current amount of hours/shows, you could make swiping your finger left or right move you 8 hours later. So swiping would load the next 8 hours of TV, and again, and again as the viewer keeps swiping. You could also shift it to Morning, Afternoon, Evening and Night for each swipe which may or may not work better.
To fit in extra icons down the bottom for sections, you could do what iPod.app currently does with a 'more' button instead of settings, which I'm guessing most people only use once. That way you could include dmw's 'what's on now' and 'what's on next' buttons. and people could customise the lower bar. I for one rarely use 'Popular' and 'My Shows' and would love a 'what's on now' to always be visible.
I'm hesitant to mention this, as it could ruin the current clean look of IceTV.app but colour coding of the TV stations could work really well, if you used subtle darkish colours it shouldn't look too weird but I think a lot of trial and error would be required.
Slight changes to those colours could represent the digital or none digital versions of the channel. In the absence of logo's (no logos, please, too messy/wasteful), colours could help separate each channel making it easier to view quickly (it's probably better not to do this, but still worth thinking about it).

While not exactly the same... IceTV have just announced three new RSS feeds: My Upcoming Shows, TV guide search and My Upcoming Shows search. These could be used to trigger alarms etc.
My Upcoming Shows RSS: This allow you to view the shows you have scheduled to record or set as favourites for the coming week.
TV guide search RSS: You will be notified when shows matching certain keywords you set appear in the next 7 days, allowing you to then schedule them to record.
My Upcoming Shows search RSS: View the shows you have scheduled to record or set as favourites for the coming week, filtered by a keyword.
