I didn't have it on mine, Optus iZoo is just a website that you can get to through settings (pretty deep), and you can add it as a Web Clip.

I think it might have something to do with whether Optus activate or "unbrick" the phone instore or whether you do it yourself through iTunes at home.

It's just a webclip to Optus iZoo. They did put another webclip when they unbricked the phone, but the link didn't work so I deleted it.

If it's anything like NZ, the carrier name is usually determined by your SIM card -- carriers do change them.
My old SIM card used to say Vodafone NZ, my new one says VodaNZ.

That's my touch. How fascinating.
I must say though, this is nowhere near as interesting as desktops. At least desktops have images and stuff. The only point to this is seeing where people have placed various icons.
Also, it helps if you actually have something to contribute to a thread that you've started.

Derailing thread for a sec.
I'm going to make some webclips, that use Javascript, to make my iPod Touch "look" like an iPhone (while on the springboard). I've had 50+ people ask me if it was an iPhone.
