+1 Crumpler - I carry my "Ripper Tripper L" everywhere. Holds all Mac laptops (not at once) my DSLR and 2 lenses, spare batteries, cables, iPhone, wallet, has a document section and still has enough room to carry some lunch.
It is, however, a backpack - though you'll be glad of that when you carry that lot around with you for any period of time! Oh and the bit between you and the laptop is padded extremely well.

My vote goes to Booq bags.
I have the backpack version for the 15" MBP and use it everyday,
it's brilliant, well built and plenty of space. I have even fit my digital slr body and two lenses in there quite fine as well as iPod etc, (not my laptop too though, but probably could of fit it as it goes in the very back vertical pocket that is good storage for documents/magazines when you don't have the laptop in there). I had the Booq slip case for it also, but it got stolen by a girl I knew so I never got to use it, however, I find that the bag offers a lot of padding on its own and I usually wrap the laptop in a soft t shirt and then slide it in the bag in the proper area and that seems to be very safe in there like that)

Someone may have said this already, but I have a Lowepro Rolling Computrekker AW, which carries all my stuff when I travel:
DSLR,
Laptop,
iPod.
Has extensible handle and wheels (great for the airport).
There are smaller ones available, and they are excellent quality bags.

I guess I'm another crumpler person. I just like their bags and the way they look. I've never had one deteriorate on me and I've got about 5 different ones.
The one thing I really like is that you can buy one big bag and then mix and match different inserts for different things. Need a camera bag? Use the slr and lens inserts. Laptop? Use the sleeve. When you just need the space you empty it out and there you go.

Another Crumpler person. Apart from their funky website and advertising, their bags are sweet. For my use, I have a 15" 'The Considerable Embarrassment'. It holds my MacBook, books, phone, folders, diary and more stuff...
I also plan on getting 'The Keystone' for my camera gear soon.
Another bonus to Crumpler bags is that they are made to last. When you open it and see the quality, you know that the logo won't peel off after a few months.
If it fits your needs and you like the designs, you can't go wrong with a Crumpler.

The Salary Sacrifice. Well worth it - but I picked mine up for $100 from the forums. I've dragged it on Geology Camp, through volcanoes and mines, carrying a bunch of stuff. Highly recommended. Hasn't frayed anywhere yet, (my stm shoulder bag did after 1 week of use...not happy!!)

P'raps I can consider a crumper (THE BASTARDS).
I really hate to but silly people like Oakley aren't making their bag anymore and it looks to be the only shoulder bag that can conveniently take a laptop, misculaneous other shit, and an SLR.
I'm just puzzled as to which bag it actually is.
